Experiences with SCORM packages

All LMS have several problems with SCORM packages. The problem are on all sides: authoring system, creating packages, understanding the package from LMS side.
I'm interested to hear from other users what experiences they made with packages from different authoring systems and what typical conflicts are.

What works well?

What creates typically conflicts?

What about huge packages with 400+ MB and thousands of files included?

What about packaging problems?

What about SCORM2 packages?
